• Fully complete Child Dedication request form with correct contact information. Baby dedications are scheduled for 2nd Sundays (excluding Holidays; i.e. Mother’s Day; Easter; Pastor’s Anniversary) and 5th Sundays.

• You will be contacted to confirm the date you have requested.
