Mount Zion Facility Brochure

This brochure is an overview of rental spaces available. Information includes descriptions of rooms, size and capacity.

Facilities Reservation Form

This form is an application for all external (non-wedding) events. It details the purpose and needs of the event as it relates to Mount Zion being the host site.

Media Request Form for Weddings

This form is used to submit ALL media needs and special requests for wedding ceremony and/or reception including reservations for a sound technician, musician, etc. This is also the form to submit requests to play specific CD tracks. Please note that ALL music played during the ceremony must be submitted and approved by our Minister of Music beforehand.

Wedding Application

This form is the application and contract needed to book a wedding at Mount Zion. Please note that the prices listed in the contract are valid through 12/31/09. The wedding must take place by 12/31/09 in order to receive these prices.

Wedding Policy

This document details the policies and guidelines of weddings being performed on Mount Zion's campus.

Pre-Marital and Marital Counseling Form

This form must be submitted with the Wedding Application at least four months prior to the wedding date if:

• the Bride and/or Groom are members of Mount Zion Baptist Church and desire to use the facilities for the wedding.

• the Bride and/or Groom desire to have an ordained Mount Zion pastor or minister officiate the ceremony.
